What are floaters, and why do they develop?

The Korean term for eye floaters, bimunjeung (飛蚊症), translates literally to 'flying mosquito symptom'—combining the characters for 'fly' (飛), 'mosquito' (蚊), and 'symptom' (症).

True to its name, it refers to seeing shapes resembling mosquitoes, cobwebs, or strands of hair drifting across your field of vision.

The causes of floaters can generally be divided into three categories.

Cause 1: Eye Aging

The most common cause is the natural aging of the eye.

The inside of our eye is filled with a clear, jelly-like substance called the vitreous humor.

As we get older, this gel undergoes liquefaction, gradually turning into liquid.

During this process, parts of the gel clump together and cast shadows,

and when these shadows fall onto the retina, they appear as floating specks before your eyes.

Cause 2: High Myopia (Severe Nearsightedness)

Floaters appearing in younger individuals often have a different cause.

In many cases, they are due to high myopia.

With severe nearsightedness, the eyeball becomes elongated from front to back compared to an average eye,

which accelerates degenerative changes in the vitreous humor at an earlier age.

Cause 3: External Trauma or Strain

Lastly, external factors can also play a role.

A strong impact or injury to the eye can agitate the vitreous and trigger floaters,

and excessive eye strain from prolonged smartphone or computer use can make you significantly more sensitive to them.

Here, many people ask:

'Is the afterimage lingering in my eyes after looking at the sun or a bright light also considered a floater?'

No, that is not an eye floater.

That is merely a temporary phenomenon caused by intense light stimulating the photoreceptor cells on the retina, and it fades quickly.

On the other hand, genuine floaters follow your eye movements constantly, drifting wherever your gaze shifts, regardless of whether you look at bright light or not.

They are particularly noticeable against a bright blue sky or a plain white wall.

This is because the brighter and clearer the background,

the sharper the shadow cast by the vitreous debris onto the retina becomes.

It works on the same principle as making shadow puppets under a bright light—the brighter the source,

the clearer and more defined the shadow appears.

Are eye floaters harmless?

Not necessarily.

While the symptom of seeing something floating is broadly called floaters,

the underlying cause can paint a very different picture.

Floaters are broadly classified into two categories:

physiological floaters caused by natural aging,

and pathological floaters caused by underlying eye diseases.

Although the symptoms may appear similar, the consequences are vastly different.

Pathological Floaters Cause 1: Retinal Tear or Retinal Detachment

A primary cause of pathological floaters is a retinal tear or retinal detachment.

This occurs when a hole or tear forms in the retina,

often accompanied by a sudden surge in the number of floaters from a few to dozens or hundreds, or by flashes of light (photopsia).

Pathological Floaters Cause 2: Uveitis

Another possible cause is uveitis, an intraocular inflammation.

Beyond just floating specks, uveitis typically presents with

eye redness, ocular pain, and a noticeable drop in visual acuity.

Pathological Floaters Cause 3: Vitreous Hemorrhage

Finally, another dangerous condition is vitreous hemorrhage.

This happens when blood vessels inside the eye rupture and bleed into the vitreous,

which patients often describe as 'a dark curtain or a dark cloud descending over my vision.'

In particular, if you have diabetes, hypertension, or high myopia and suddenly develop floaters like this,

there is a high probability that it signals a serious underlying condition.

If you notice floater symptoms, it is advisable to visit an eye clinic promptly.

While simple physiological floaters do not directly impair vision,

pathological floaters can lead to irreversible vision loss if treatment is delayed.

Therefore, whether the shadows you see are harmless physiological floaters

or symptoms of a condition requiring urgent intervention,

undergoing an in-depth examination by an ophthalmologist is essential.

Why do floaters develop at a young age?

Many younger individuals are taken aback, thinking, 'Why am I seeing these specks when I'm still young?'

There are two main reasons for this.

Younger-Onset Cause 1: High Myopia

The first is high myopia.

With severe nearsightedness, the axial length of the eyeball is longer than normal.

Because the vitreous volume is limited while the interior ocular space expands,

a pulling force (traction) is exerted by the vitreous against the retina.

During this process, the vitreous may detach prematurely from the retina or form aggregates,

leading to floaters even in youth.

Younger-Onset Cause 2: Relationship with Vision Correction Surgery

The second factor is the connection to vision correction surgery.

Procedures like SMILE, LASIK, or LASEK do not directly create floaters themselves.

However, patients who previously had high myopia often find that once their vision becomes sharp after surgery,

they notice existing, previously blurry floater particles much more distinctly.

Additionally, the temporary pressure applied to the eye during surgery may accelerate vitreous changes,

making floaters noticeable sooner.

Importantly, because of this traction force in younger individuals,

there is a relatively higher risk of developing retinal tears or holes.

Therefore, rather than assuming you are safe just because you are young,

it is strongly recommended to get your retina examined if new symptoms arise.
